(Undated) -- The Occupational Safety and Health Administration is looking into two deadly accidents at Amazon warehouses. The first happened last December when a man was pulled and crushed on a conveyor belt in a New Jersey warehouse. OSHA cited five companies for "serious" labor violations, but did not cite Amazon. The second happened June 1st at a warehouse in Pittsburgh. A woman was killed while moving pallets with a motorized jack that slammed into shelving and pinned her underneath. Amazon says it's cooperating with OSHA investigators.
